The Vice-minister of Healthcare and social development of RK Yelzhan Birtanov informed today at briefing in the Central Communication Service of the president of RK for topic "About modernization of the system of healthcare of Kazakhstan".
In the beginning of his appearance he reminded, that on January 15, 2016 by the Head of state was approved the State healthcare development program of RK "Densaulyk" for 2016-2019, implementation of which will be carried out in the following directions: development of social healthcare as basis of protection of population health; integration of all health services around requirements of population on basis of modernization and priority development of primary medical-sanitary aid (further PMSA); provision of quality of health services; implementation of National drug provision policy; refinement of health care system on basis of intervention of solidarity and enhancements of its financial sustainability; increase of efficiency of human resource management in healthcare branch; cover of the further development of healthcare infrastructure on basis of state-private partnership, innovations and modern information and communication technologies.
"In accordance with the best international experience formation basis for the further health development will become the Social Health Service (further SHS). The main functions of SHS will become: education and propaganda of healthy lifestyle among population; monitoring of infectious diseases and the major non-infectious diseases, including, during violations of mental health and traumatism; ensuring inter-sectorial cooperation, aimed at protection and strengthening of population health of the country; control over compliance with legislation in the field of health protection and epidemiological safety, long-term forecasting of development of diseases at regional level and national level» – Yelzhan Birtanov reported.
In the words of the Vice-minister, special attention will be given to health improvement for school-children. In that regard, transfer of school medicine from education system to healthcare system is provided in the framework of the State program "Densaulyk". Development of the Calendar of sanative events for school-children is planned during all the year which it envisages: directly conducting of professional examinations of children in educational institutions; improvement of school-children during all the year; introduction of health saving technologies in schools, aimed at disease prevention.
PMSA will become the central link in system of medical care organization for population with change of its interaction with horizontal ones (ambulatory-ambulatory ones) and vertical (psychiatric, narcologic, anti-tuberculosis, oncological) and other profile services. Expansion of stage volume of medical care and list of medicine instruments for free outpatient treatment is planned in the framework of PMSA.
The vice-minister noted introduction of the Program of diseases management (further PDM) was one of the main directions of State program of "Densaulyk". It will provide a possibility for the patients together with medical workers to manage their diseases and prevent complications by continual contact of much discipline team with a patient, a patient's self-help delivery skill training, as well as by control of factors influencing health.
Since 2013 PDM in a pilot regime has been introduced in 7 polyclinics of the Northern Kazakhstan and Pavlodar oblast in three nosologies: arterial hypertension, chronic heart failure, and sugar diabetes. Taking into consideration positive experience, in the period from 2016 to 2017 PDM will additionally be introduced in five regions, since 2018 – in all the areas of the country.
In addition, an integrated model of delivery of medical care with the major socially significant, non-infectious diseases and diseases significantly affecting demography of population of the country will be introduced in the framework of State program: acute myocardial infarction, acute disorder of cerebral circulation (stroke), malignant tumors, traumas, pregnancy and obstetrics. All the series of services will be delivered in each direction: prophylaxis, therapeutic events including a rehabilitation, clinical observation, medical and social services.
Yelzhan Birtanov said, having added in the commission's work representatives of the NGO would take active part, "Creation of the Joint commission on the quality of health services whose activity is aimed at development of recommendations on refinement of clinical protocols, the standards of medical education, drug support, the standards of quality control system and availability of services in the field of healthcare became the new institutional mechanism for quality improvement of delivery of medical aid".
The vice-minister also reported, that in the framework of implementation of National drug provision policy in 2015 had been developed the National drug form including all the list of drug remedies registered in Kazakhstan, information about their proved clinical effectiveness, possible side effects and the limit of price on each drug. A transition to the standards of GXP (the proper pharmacy practice in sphere of imposition of drug means are production, transportation, storage, sale) from 2018 will become the next stage of National drug policy.
For purposes of provision of financial sustainability of health care system since 2017 social mandatory social health insurance (further MSHI) will be introduced in Kazakhstan. The state will realize provisionings for 15 categories of citizens, 12 of them social-vulnerable segments of population. Employers – for employees. The workers and the self-employed citizens registered in tax authorities are for themselves.
Nowadays the Road map is developed and approved on introduction of MSHI in accordance with which this year will be developed normative and legal bases and created socmedinsurance Fund (further SMIF). It allows to begin fund-raising since January 1, 2017 and make since July 1, 2017 paying of health services through FSMS.
For cover of sustainable health development strategic human resource management will be carried out by introduction of new functional and qualifying requirements to the managers of medical organizations, perfection of a system of personnel resource planning with consideration for real requirements of population, introduction of independent estimate of knowledge and skills of medical workers with involvement of representatives of professional associations and non-governmental organizations.
"Development of medical and sisters' education will envisage triune of clinical practice, medical education and scientific activities. For this purpose a concept of development of integrated medical academy centers (university clinics) will be developed in a way of coalescence of scientific organizations, medical universities, inpatient and ambulatory institutions on the principle of complete cycle of health services and staff training. Creation of new clinical bases and university clinics will be carried out in strategic partnership with the leading international universities and medical organizations. Attraction practice of the leading foreign experts, leaders (managers), scientists and teachers to academy trials will receive broad development,» informed Yelzhan Birtanov.
The further reform of healthcare system will be based on formation of infrastructure, flexibly reacting to the requirements of population for available, full value, qualitative health services. In particular, it is planned to hold optimization of state healthcare infrastructure by horizontal and vertical integration and merger of medical organizations. Possibility of stage-by-stage transformation of region and republican organizations along with it will be considered, including higher schools and scientific centers in autonomous healthcare organizations for experience of AEO "Nazarbayev University".
In addition, it is planned to design and introduce the new system of the standards of healthcare infrastructure on the basis of the OECD standards, as well as standards in the area of resource accumulation, energy efficiency and eco-friendliness subjects of healthcare.
Also envisaged by the State program "Densaulyk" creation of the National medical technology database, implementation of service contracts of the public private partnership, development of private leasing.
Integration of healthcare information systems and resources will be carried out in the framework of creation of Unified information environment, introduction of hospital information systems is envisaged, own health information access of patients by means of electronic passport of Healthcare and patient's personal cabinet is provided, as well as IT tools for the effective activity of FSMS and a smart-medicine on the basis of the public private partnership are introduced.
It will allow to automate all work processes in medical organizations and keep accounting of medical information in electronic format.
An independent online patient enrollment for acceptance to doctors, counseling in a real time mode, and their observation with use of personal static computers and smartphones will also be introduced based on stage.
By 2019 coverage of population in electronic passports of Healthcare and share of electronic hospitalizations will reach 50%, share of health organizations connected to Unified network will reach 80%.
